Record detection time, response, restored scheduling and whether the gap affected recovery objectives. If a recurring failure remains unresolved, show it in the operating review. A quiet dashboard should mean the monitored conditions are understood, not that all alerts have been turned off.

Write the acknowledgment separately from the repair

Fictional alert record: failure detected at 08:10, responder acknowledges at 08:20, cause remains unknown, next check owned by the database operator. Acknowledgment prevents silent neglect but is not restoration of protection. Once a supported correction is made, record the next successful job and assess the gap left in the recovery history. Keep response targets realistic for staffing and holidays. The useful operating question is how long a meaningful failure can remain unresolved, not how many notification integrations appear on a feature list.
